·         Cardiomyopathy, congestive heart failure.

·         Hypertensive heart disease.

·         Valvular heart disease.

·         Infective endocarditis.

·         Renovascular disease, peripheral vascular disease.

·         Evaluation of chest pain/ shortness of breath/ leg swelling/ tiredness/ palpitations/ loss of consciousness.

During a visit:

Please bring the following information to your appointment:

·         Insurance card(s)

·         Insurance referral (if required)

·         A list of all medications (and dosages) you are currently taking

·         All previous health records espescially cardiac records and reports if any available

·         Primary care physician information

 

Prescription refills:

Please call our offices.

 

Emergencies:

Please call 911 or go to the nearest emergency room and mention "Longwood Cardiology" as your cardiac care provider.

 

For urgent care:

Please call the office as most same day appointment requests can be accommodated.
